
Daniela Maria Zajac-Pavlik, a longtime resident of Warwick, NY, passed away on May 10, 2016 at Orange Regional Medical Center. She was 60 years old.
Born on October 4, 1955 in Velke Rovne, Slovakia, she was the daughter of Ludmila (Kubenik) Zajac and the late Henry Zajac. She came to the United States in 1969.
She was married to Jan Pavlik.
Daniela was a Registered Nurse, specializing in Neonatology and worked with the Pediatric Group at Westchester Medical Center, Newburgh. She formerly worked at the Methodist Hospital in Brooklyn and Long Island College Hospital in Brooklyn.
She was a member of St. Stephen Church, Warwick, where she helped organize Holy Hour, Adoration and Divine Mercy at the church. In addition, she volunteered at Schervier Pavilion with her daughter Kateri with masses.
She is survived by her husband, Jan Pavlik of Warwick; her mother, Ludmila Zajac; her daughters: Stephanie Pavlik of Naples, FL and Kateri Pavlik of Warwick; her sister, Bozena Kovac and her husband Paul; her brother, Lubo Zajac and his wife Eva; and several nieces and nephews. She was predeceased by a son, Jan Paul Pavlik (1997).
Visitation will be on Saturday, May 14th, from 1:00-4:00 pm and on Sunday, May 15th, from 4:00-7:00 pm at Lazear-Smith & Vander Plaat Memorial Home, 17 Oakland Ave., Warwick. Funeral Mass will be at 11:00 am on Monday at St. Stephen RC Church, Warwick. Interment will follow in Warwick Cemetery.
